18F-Fluorodeoxyglucose Uptake on Atherosclerotic Lesions in Patients with Acute Ischemic Stroke Evaluated by Positron Emission Tomography

Abstract

|
Background: The vulnerability of plaque rupture, most important features of atherosclerosis related to the thromboem-bolism, is depicted with the characteristics of the plaque, particularly the degree of inflammation. Recently, some studies have reported that 18F-fluorodeoxyglucose (FDG) positron-emission tomography (PET) appears to be capable of identifying and quantifying the degree of inflammatory reaction in atherosclerotic plaques. We hypothesized that if the atherosclerotic plaques were responsible for thromboembolism in stroke patients, then it would show the evidence of inflammation on 18F-FDG PET images.
Methods: Sixteen patients with the steno-occlusive lesions in proximal carotid artery or huge plaques in aortic arch were underwent 18F-FDG PET fused with arterial enhanced-CT scan. We measured the uptake activity of 18F-FDG on all of the plaques identified on enhanced-CT fused with PET, and compared with those of normal appearing arterial segments documented by ultrasonographic examination in each subject.
Results: Patients with symptomatic plaques showed higher 18F-FDG uptake than those with normal appearing arterial segment (2.63¢®¨ú0.94 vs. 1.78¢®¨ú0.30, P<0.05 by Kruskal-Wallis test).
Conclusion: The 18F-FDG PET fused with arterial enhanced-CT scan could assess the degree of inflammation in the atherosclerotic plaques and could detect responsible lesions for embolic events or ruptured plaques.
